I've never truly felt confident in my intelligence, so tests always give me major anxiety.

Here's my rule for worrying about stuff:
Can you do anything to change it?
If yes, then do it and you won't need to worry.
If no, then there is no point in worrying.
For example, when you give the paper with your answers to the professor, there is nothing you can do about it, so just relax and wait for the results.
Also, consider what would happen if you fail the test. Usually you can just retake it. Most likely you will not get shot for failing it. So, if you fail, then fail, next time it will be easier and you will pass the test.
At least this is how I approach various tests.
