Grown up Henry is reunited with his Cinderella and Lucy

Wish Realm Henry, lost his want for revenge against Regina, and gets adopted.

Wish Realm Hook's poisoned heart is cured by Main Rumple litterally giving his heart to him.

As a result, when Our Rumple died, he died a hero and is reunited with Belle in the afterlife. While his eviler counterpart, Wish Realm Rumple turns to dust into the fire

In the final scene, All the magical fairy tale lands are brought together like United Realms. And to Regina's surprise, they all elected her to be their Queen. Snow White was especially pleased to crown her the Good Queen she felt Regina could be.

Even Emma and Hook arrived just in time to see the coronation.

by the way I couldn't help but notice the parallels between Once upon a Time and My Little Pony friendship is Magic in terms of character and some plot points. (Twilight is Emma, Sunset is Regina, Discord is Rumple, Time Travel was involved, a villain turned to stone, a huge time skip happened)
