Claptrap from Borderlands comes to mind. But he was intentionally made to be annoying so I'm not sure if he counts. Scrappy Doo gets a vote from me too, I used to find him so dang annoying as a kid, especially in that live action movie. Also Jar Jar Binks, of course. I do feel bad for his voice actor because of all the hate he received from fans due to Jar Jar being so disliked.

Dolores Umbridge, Lili Potter and James Potter (Harry Potter series). Umbridge is too irritating, and even though the point is to make the audience happy when she gets her comeuppance, she doesn’t get nearly enough to justify hours of her unrestrained irritation onscreen or in the books. Lili and James, well, they suck and I’m glad Voldemort nuked them both.
