Animators have always included little jokes for their own amusement... Disney films are littered with in jokes... pixar are always including Easter eggs! A113... So it's not as creepy as a faceless pony in the bushes but not many people know the story behind it... You only REALLY spot it if you're looking for it...

On Twitter, McCarthy expressed that she had to look up what Slenderman even was after the episode. This suggests to me that this easter egg was thrown in by one of the animators, not the writers or directors. Make of that what you will.

 

Personally, much like the Grumpy Cat cutie mark (which also seemed to be added post-production due to the janky way it moved), I find it to be a played-out reference and rolled my eyes a little at its inclusion.

Thank you very much, this information really means a lot to me. So it was the initiative of one of the animators, not across the studio in general
