Click your name on the status bar.
My Settings in the dropdown menu.
Display Name in the menu to the left.
Enter new name.
Profit.

Need to have a URL to the image, then you just use the image button in the toolbar of the text box. So if the image is on your pc, upload it to photobucket or something so you have a url, otherwise just copy the url for the image from your browser. Though I'd recommend uploading it to photobucket still and linking from there just in case the site currently hosting it ever removes/changes the link.
