Sharkpony worked in Ponyville General alongside Doctor Horse for many years. Despite her looks, she was a true professional and would only eat ponies who looked like turtles from below, which was only ever the odd pegasus. She knows that if she ever stopped treating ponies or drinking coffee she would just die of boredom, so she works all hours. Her favourite patient is Silverstream in seapony form and her least favourite is Fluttershy, because it's so hard to get her to explain what is wrong. She has no love of her life, but she does have a pet parrot called Serro and a chihuahua called Tonin.

Derpy Hooves humors a foal named Emerald Murmer at the Ponyville Hospital. Derpy is uneasy because she thinks her heartbeat has been sounding funny all day while doing her sorting at the Post Office, and the actual doctors and nurses are preoccupied with other patients at the moment, so she's relying on the prognosis of a child.

Emerald is visiting her Aunt, Nurse RedHeart, for the weekend to learn what it's like to work in a hospital after recently obtaining her cutie mark of a stethoscope shortly after first aid training at summer camp.

She finds nothing immediately wrong with Derpy and escorts her back to the post office, where upon Derpy immediately picks up the sound of her heart beating funny, only Emerald can hear it too! It turns out they acquired a newly automatic printing press that had been running unsupervised nonstop in the basement mailing room, which was now filled knee high with loose sheets of paper. Derpy had simply mistaken it's repetitive rhythm echoing off of the earthen walls as an internal matter.

The question then remained over how the machine turned on. A rustling from the sea of parchment gave way to a yawning Mr.Waddle, a local volunteer who came to make copies of bingo cards for that nights senior citizen tournament, but had fallen asleep during the task.

Derpy Hooves was thrilled to be medically cleared, and hopes to get the mess cleared up before her boss returns. A proud Miss Murmer returns to the hospital to find her aunt wrapping up her shift, and exclaiming she had wondered where her niece had run off too. She would laugh at the tale of the little adventure as they made a stop at Sugar Cube Corner before going home for the day.
