Besides the super obvious bad voice acting/controls/plot/etc., here's my list:

 

Extremely powerful comeback mechanics - I'm talking about things like Street Fighter IV's ultras and Marvel vs. Capcom 3's x factor. I know, comebacks are exciting and cool, but I think touch of death combos are a bit much.

 

Lack of direction - I get it. You want the player to be able to do whatever they want, but without some kind of goal or objective to accomplish, I find myself wondering why I should do anything at all. I run into this problem with those games that have really short main plots but tons of sidequests, which, while interesting to some degree, aren't long enough to really draw me in.

 

Unskippable cutscenes and tutorials - I've already seen this/ already know this. Just stop.

 

Excessive (ab)use of quick time events - Now, I don't mind QTE every now and then; I think they look nice and cinematic while still retaining a small amount of player input. I think some games pull it off well ( I like Resident Evil 6's, for example), but for other games putting in a ton of QTE makes them feel really forced and very annoying.

 

A party of generics - I'm talking RPG's where anyone in the main party can do anything and everything just as well as anyone. The biggest example I can think of is Final Fantasy XII (not the international version). None of the characters have any uniqueness save their appearance, very boring. 

 

Sudden and steep difficulty spikes - I'm playing through the game at normal pace, don't really have to do any grinding, the battles along the way to the boss give just enough exp to comfortably beat the boss. I continue the story, get to the next area and suddenly the mobs/boss are/is destroying me. It's just weird to have to do a ton of grinding all of a sudden with no warning whatsoever. (Optional bosses and areas are, of course, exempt from this)

That's where me and 90% of gamers come to a disagreement on Morrowind, I can't seem to really get into because of the lack of guidance on where I need to go and would love someone to mod a compass into it.
