Need help filling out a W-8BEN!

So me and my friend have been offered to become a YouTube Partner as a part of the Pixel Enemy Network. As part of the process, we need to fill out a W-8BEN. I hope any of you Bronies have some knowledge in how to fill in such a form, because we sure don't!

 

I've been following a guide in order to fill it out, but there's still a part I'm confused about:
