A whole new game mode, A new character, more than 100 new items, new alternate versions of floors with more enemies and bosses, 8 new transformations, almost double the challenges, new endings, tons of secret stuff, a daily challenge kind of thing, more synergies with item combos, etc.

Cool CAPTN. If you liked that you might like a few other of his games. "Aether" and "Spewer" are also both pretty fun.

 

 Isaac's still my favorite though, there's a demo of the flash version of Isaac on Newgrounds too, though Isaac's one of the games that get's into a lot darker territory.

Health Insurance Portability and Accountability Act of 1996.

 

Basically it determines what sensitive patient information is (name, address, age, health conditions, etc) and lays out strict fines and punishments for people working in healthcare. 90% of the time, if you work in a healthcare facility and you're in violation of HIPAA you will lose your job.

By this point, between the Halloween update, Wrath of the Lamb, being remade into Rebirth, and Afterbirth the original game has more than tripled...possibly quadrupled in size.

 

 Afterbirth's supposedly the last though, at least until that hypothetical Isaac sequel.
