I told you! It's his mind, his imagination, most of the monsters are distorted mirror images of himself.

 

 Or maybe that was Isaac. The powerups all change his appearance. You might have just called an innocent child a monster.

Isaac can look rather horrifying by the end of the game. Most of the passive items change his appearance, and a lot of them stack. You could have a four-eyed, belt wearing, demon horned Isaac with angel wings and a battery in his head. Or any other number of looks.
