B. If the user removes the video him/herself

C. If the user account associated with the video is terminated

D. If the video violates YouTube's Terms of Service

I've seen a lot of A happen, especially with countdown makers and people who post Pokemon Z-Moves in videos.

B happens fairly rarely it seems like.

C happens more often than B, but it's rare and usually for decently good reason unlike A.

D happens, but it almost always just gets demonetized most of the time when something like that happens (A LOT, considering they seem to change them constantly).
