 In the original there's an item called "My little unicorn". It's a unicorn horn that makes Isaac temporarily invincible when he uses it.

 Isaac is filled with references to other games and cartoons and other things. It makes sense. The entire game takes place in the imagination of a little kid. No wonder it would have all that stuff floating around in there.
