How tolerant are their immune system? In it, Twilight Sparkle got sick from a fungus-based food poisoning that affected almost all of Ponyville. She mostly rested in bed while Spike and Zecora teamed up to find a way to cure it. Scan. Not quite the same thing, but theirs a number of times where they got cursed or inflected with weird "magical diseases", like in "The Night of the Living Apples", "Ponies of Dark Water" and "Chaos Theory"/"Accord". Otherwise, Twilight and Cadence wouldn't have needed a magic health bubble to protect them from the blue flu. If they did get sick, I expect they'd just take some time off. I'm sure Equestria can survive a few days without one of the princesses. After all, there were technically only two princesses before Season 1, and now there's four! It wouldn't be practical for her to say go all the way north to the Crystal Empire if Cadence is sick in which case it'd be Shining Armor but he'll probably want to help take care of her since they're married and all and there's the baby to consider. Twilight could be called up since she's more available than either of the royal sisters but she's even further south and still has duties of her own so for all we know Flash Sentry could be put in charge for a few days.
