Caramel, in a way, sounds like she doesn't know Starburst as she did in the other timeline, Starburst remembers her and what they did together, Caramel doesn't appear to know.

I was thinking that the medication that they're giving her is actually causing Caramel to be weak so they could blackmail Grand.

Fire has Caramel, Stephan will make sure they aren't found out for as long as he can. He'll then follow Fire, then they find out what's going on and hide atop the town hall at an open window waiting for the right moment to strike.

 

 

Maybe when Starburst sacrifices himself to save Caramel, she remembers who he is and what he means to her.
