The fans had assigned Derpy has the Doctor's companion, but if that is real then I suppose Colgate (Minuette) is his companion.

In Doctor Whooves and assistant, he travels with Derpy/ditzy

In Doctor Whooves Adventures, he travels with Twilight, Spike, and sometimes Pinkie.
