There are two that stand out among the many for me.

 

Thulsa Doom from Conan the Barbarian.  Not physically menacing perhaps, but he had such an insidious 'niceness' about him that just made him even more terrifying.  Also, the casual, offhand way that he ordered one of his own followers to leap to their death for no other reason than to demonstrate his power to Conan was chilling.

 

Khan Noonien Singh from Star Trek 2 The Wrath of Khan.  Every line he speaks is quotation gold, with just the right amount of crazy thrown in for good measure.

 

Other notable mentions would include,

'The Kurgan' from Highlander, just for the voice if nothing else.

Darth Vader, because Darth Vader.

Keyser Söze from The Usual Suspects.

 

I could go on, maybe later.

I love Loki, Ultron, and Ronan The Accusor (from The Avengers, Avengers: Age of Ultron, and Guardians of the Galaxy). Although Ronan didn't have too much character development, he still made an awesome bad guy. And whilst I tend to prefer Marvel movies over DC productions, the Joker (The Dark Knight) is one of my favourite villains. I also agree with the OP about Agent Smith! And Darth Vader is a legend.
