 I'm actually talking about an Isaac item. When you shoot them they just hang still in air until you release them, then they all launch forward in the direction you aimed them. It's a really cool item.

Once you've picked it up it stays with you for the rest of that run. It affects all of your tears.

He uses his own tears to fight? That's both creepy and efficient.

His tears can be changed throughout the run too. So you can have tons of possible effects. Like giving them poison, fear, or slowing effects, increasing their size, making them turn enemies to stone, or spin around him in an orbit.

 

 Of course there are other possible "Weapons". The Brimstone item makes Isaac launch a giant beam of blood from his mouth instead.
