well, if some stuff is really a traumatic reason(One of the dark themes in MGSV) from Big Boss, like those in the hospital, it could be true that some stuff is truly not existing.

 

Kinda like in Uncharted 3, when Nathan drinks from the poisoned water and suddenly imagines fire demons he has to fight and his best friend die, but all never happened truly

 

Or in COD Black Ops during the game, Alex is often together fighting with the russian Viktor Reznov he meet in a prison, but all the time, he just imaginated him or he itself saw himself as Reznov, so he was alone but imagined him. But his brain was just manipulated.
