In Super Paper Mario, we had places like the World of Nothing and The Underwhere (the latter had the creepiest place in it, River Twygz).

 

And of course there was that deformity called Super Dimentio...

"The Library" from Halo: Combat Evolved.  You were fighting the Flood forms, which you can get pretty jumpy since they jump right at you.  Not to mention, its the longest level in the game, yet the most annoying.

Those things scared the shit out of me when I was a kid. Trying to get away from them was a nightmare! It doesn't help that they can't be killed. That music was also pretty damn creepy.

 

This was the first level I ever played in a video game that actually scared me.

When I was a kid, all of Lavender Town and especially Pokemon Tower creeped me out. I was 8 years old and I first heard that song... Ugh. Then I walked into a Pokemon graveyard full of ghosts. Fuck that, I'm out!

Uh, yeah the whole level was creepy to me, as a kid. :c

I could list so much, but this tops it all. That horrible piano scared me to death as a child. My first ever video game introduced me to.. killer pianos that chase you around the room D:

Lavender town also creeped me out xD Still kinda does, but now I can listen to the music and wander through town without a problem. What scared me the most was the girl who asks "Do you believe in ghosts?" "So that white hand on your shoulder, I'm just imagining it." o.o
