As I've told you before, practice more self-compassion and when those bad thoughts come up challenge them. Are you useless? No, you're not, people care about you and want you to be happy.
Are you fat? Just as long as you're keeping up with your health, you should be fine and if you don't like your body right now you can always try losing weight if it's bothering you that much. Keep in mind that muscles need some fat, bodybuilders look totally different when they're actually at a competition compared to when they're not competing and actors with shredded abs are on highly restrictive and dangerous diets just to look good for a few shirtless scenes in whatever movie they're in. I can send you some good workouts you can try in the comfort of your own home that I find useful for me if you were interested? It might feel kinda silly doing them at home but you'll get over it, there's also loads of workout videos on youtube you could try. Just remember, losing weight mostly depends on a good diet, you can't outrun a bad diet. Talk to your doctor about it, they could help you get on track with losing some weight if you wanted to.
Are you ugly? Dude, you're just not your type, don't beat yourself up over it. I'm sure you'll find someone if you put yourself out there and go to meetups and stuff like that, you know? You can't catch fish if you don't go to the sea, right?
You really do need to be kinder to yourself, I'm not saying you should focus on thinking positively all the time that's hard to do. When you get those negative thoughts, try to make a list of what supports it and what doesn't support it.
