(Or at least that's how I see it.) Definition 1: A 3D model with a low number of polygons. Interpretation B: A 3D model with following characteristics (non-exhaustive, not necessarily all will apply): no shading such as Phong shading; a blocky appearance where round objects, such as cylinders, will appear as prisms or irregular icosahedra (or similar); very little colour variation intended to give each polygon in the mesh one uniform colour; usually a low number of polygons...
