To visit someone at the hospital, you need to check in or have an approved reservation during scheduled visitor hours. Twilight didn't sneak Flurry Heart or Spike in for that matter into the hospital, so it's clear the staff at the hospital approved what she did. And besides, how do you know that isn’t to some extent powered by magic, which can be imbued into inanimate objects?
