Account Suspended, who suspends account and who designed the orange and blue?

OK so you know that Account Suspended message that appears on a suspended website. Who issues it? and Who designed the orange and blue? does anyone know?  just curious.

When a website gets suspended, it's usually done by a third party, like when Megaupload got taken down by the government. As for who designed it, I've no clue.
