There are three distinct phases (well, maybe four):

 

1. Salvage an old 4x4 from the back and use that as the stand
2. Salvage a bunch of old wine racks and some boards from an old table to create the height adjust arm
3. Affix a brace to the arm that will hold the actual mounts

I just recently had to redo phase 2 because the original arm was too long. Maybe some pictures will help clarify?
