So many theories! My theory is that she was dropped on her head as a baby, so It's just a lazy eye or what not.

It was an animation error of many thanks to the wonderful yet unreliable flash software used in the show, nothing more.  Her name also came out of this incident, thus it is not canon.  It has been stated that her real name is actually Ditzy Doo, this is why you keep seeing that on her from time to time and actually explains why she is such a klutz, making her less of a handicapped pony some keep assuming wrongly.

 

The fact that Rainbow Dash called her Derpy in the original cameo where she was given in Last Round Up is because Hasbro made the decision of satisfying the fans, which backfired into what the edited version is today.
