Super Mario 3D World levels are pretty creepy, but so unique.

Creepiest levels for me would probably be the Boo level in Super Mario 64 and the Egyptian level with the pyramids. The Legend of Spyro series had some creepy levels in the sense of creepy scenery, but not creepy imagery.

The creepiest for me has to be silent hill in general for the PS1.

 

The ambiance, locations, and enemies make the game scary and creepy as heck.

 

That, and while I love the intro song, the introduction and ambiance of the song is creepy as well.

My gosh! This level had everything! Mutated bunny rabbits! Knife throwers who were choking up their own knives! A final boss who is a two headed dad run through a meat grinder! Not to mention, Psychonauts was a relatively tame game until this level was dropped unexpectedly.

The Cave of the Past from EarthBound. Just the thought that you're stuck in a greyscale-style cave, with your soul transferred into a robot body in order to travel back in time is pretty unsettling, at least to me.

 

The worst part though, the music, despite being very short, does not help one bit:
