A few possibilities here:

 

1) Apple Bloom finally gets her cutie mark (though I doubt it), but this causes changes in her life that affects her relationship with those around her, especially her fellow CMCs. 

 

2) Despite the fact that they've been trying to get their cutie marks for years, none of the CMC have asked themselves "And then what?" until now. Not knowing how getting her cutie mark will ultimately affect her life ends up causing her no end of worry. 

 

3) A ton of you are saying this could very well be an Apple Bloom/Luna episode (possibly because of the reason above), which you know is inevitable at this point. Luna's visited Scootaloo and Sweetie Belle, so there is no reason why she shouldn't help Apple Bloom at some point down the road.

 

Personally, I'm going with the second or third option here.

So many people are jumping to the conclusion that Apple Bloom will get her cutie mark in this episode. Seriously, where in the episode synopsis did it hint that this episode would end that way?

By the sound of things, this will be Applebloom's long anticipated nightmare episode, so I'm on that side of the fence. On the other hand, I do think these episodes are part of an overarching story arc with a climax involving all three fillies, and maybe that will be them getting their cutie marks. On the other hand, I think it would be odd if that particular event were to occur in episode 4. It sounds more like something we would see at some point in the tail end of the season, meaning I doubt this episode will be when the CMCs get their cutie marks, but it could build up to one when they do.

I'm not sure if this is the episode, this sounds like it's just going to be some sort of reassurance episode, I think I'm seeing a pattern here, notice how in season 4 Scootaloo had to overcome anxiety issues, it seems similar here with Applebloom, also that scene with Sweetie Belle in the trailer might be a similar deal. I think what will see is that the CMC first have to overcome a major emotional issue before they can get their cutie marks.

I go with the "AB/ Luna nightmare" idea because the other 2 CMCs have had one, it's her turn.  As to getting her cutie mark, maybe.  I predict that Diamond Tiara tries to get the 1st one to break up with her "baby" friends.  Still, DT will have to think up a new insult.  Here's hoping the strain makes her skull crack open.
