I'd Imagine that AppleJack and familys Last Name is Apple, Doesn't sound great though. This is Simply due to the fact that her and her family often reffered to as the Apple family

Well in the typical tradition of country names taking the first and middle name and making a nickname (usually with the real names being something exceptionally fancy) I would say AppleJack would be something like Appollonia Jacqueline McGillicuddy. Shortened to AppleJack.
